شرح : این پرسشنامه مطالعه ای در سطح بین المللی است که با همکاری بسیاری از پژوهشگران گسترش یافته است. دست اندرکاران این پروژه درخواست کرده اند که اگر علاقمند به کار روی آن هستید، هرگونه تغییر و یا حذف می تواند کار را از اهداف خود دور کند، لذا توصیه کرده اند که برای اجرای آن با محققان ارتباط برقرار کنید. نسخه سوم این پرسشنامه نیز وجود دارد که در اختیار همکاران پروژه گذاشته می شود.
