شرح سایت روان سنجی: این پرسشنامه برای اندازه گیری توانمندسازی فردی در ارتباط با سلامت شخصی ساخته شده است.

اعتبار: هماهنگی درونی، آلفا کرونباخ برای کل ابزار 0.83 ، "مشارکت در تصمیم 0.79، "درجه مهار" 0.89 ، "مشارکت در تعامل ها" 0.79 و بازآزمایی 0.70
نمره گذاری: این پرسشنامه دوبار نمره گذاری می شود، در سمت چپ با عنوان "آیا احساس می کنید این ..." و در سمت راست "چه اندازه این مهم است..."
