7. Riding the school bus or traveling in the car.
8. Being at school all day.
9. Doing the activities in gym class (or playing sports).
10.Reading or doing homework.
11.Watching TV.
12.Walking the length of a football field.
13.Running the length of a football field.
14.Going shopping.
15.Getting to sleep at night and staying asleep.
شرح سایت روان سنجی: این ابزار برای سنجش محدودیت ها عملکردی کودکان در فعالیت های بهداشتی و اجتماعی طرح ریزی شده است. نسخه والدین نیز دارای همین ماده ها است. کاشیکار و همکاران، 2011 برای آن دو عامل ذکر می کنند.
اعتبار: هماهنگی درونی، آلفا در دامنه 0.86 تا 0.91 و باز آزمایی پس از دو هفته کودکان 0.74، والدین 0.64 و متوسط در سه ماه؛ کودکان 0.48 و والدین 0.39 (کلار و واکر، 2006)
نمره گذاری:
0 = No trouble‚ 1 = A little trouble‚ 2 = Some trouble‚ 3 = A lot of trouble‚ 4 = Impossible
